Request for Diagnostic Evaluations for Non-Public School Students

Welcome!

Thank you for contacting Seminole County Public Schools (SCPS) Non-Public Services. We are here to support families seeking diagnostic evaluations for school-aged students not currently enrolled in the public school system. For pre-school aged children who have not yet entered kindergarten, please contact the SCPS PK Disabilities Program at 407-320-9406.

What Services Are Available?

We offer evaluations to support a variety of student needs, including:

  • Initial evaluations to determine if a student has a disability and may require special education services
  • Reevaluations for students already identified with a disability
  • Gifted evaluations
  • Matrix review (only for students that have had an IEP)

How to Begin

To start the process, please click Request an Evaluation Intake Form and gather the following documents to be uploaded. Note: Only a parent or legal guardian may complete this form.

Required Documents:

  • Student's Birth Certificate
  • Parent/Guardian official Picture ID (Driver's license, state ID, passport)
  • Legal Guardianship documents (if applicable)
  • Most recent 504 Plan or IEP (PDF format, if applicable)
  • Vision and Hearing Screening results (if available)

Next Steps

Once you have completed the evaluation request intake form and uploaded the required documents:

  • You will be contacted regarding the next steps.
  • A packet requesting additional educational information will be sent to your child's private school.

Important Notes

  • A separate evaluation request intake form must be submitted for each child.
  • Please be as thorough and accurate as possible in your responses.
  • Please note that incomplete or missing documentation may delay timely processing of your request.
